Immunization Rate Reporting 2024 and Training Video
This post is to remind everyone the annual rate reporting for 2024 is due by Tuesday, December 31, 2024 and surveys are officially OPEN! You can access the childcare and accommodation facility surveys on the DPBH website. The direct child care survey link can be accessed here and the direct accommodation facility survey link can be accessed here.

The process will be the same as in previous years, but for those of you who may not have done this before or if anyone needs a refresher on how to report school rates, you can reference the training video created last year. Please share this resource with the child care facilities you work with who will be submitting immunization rates for their child care facilities. It is under 15 minutes and it is packed with a lot of great information!
Child Care Rate Reporting Template + Instructions
An Excel spreadsheet template for child care facilities has been created to make the rate reporting process a little easier this year. The spreadsheet is intended to calculate ages of the children in your facility at different times. There is also a template instructions PDF on how to use this template.
